GET YOUR COVID-19 VACCINE AT ISAK

 

Summit County Public Health, in partnership with ISAK, will be offering single-shot Johnson & Johnson COVID-19 vaccines to the community for ages 18+.

This vaccination clinic will be held at ISAK on Sunday, April 11, 2021 from 11 am to 4 pm.

Please register at isak.org/vaccine. Once registered, please download the consent form at isak.org/consent and bring the completed and signed consent form with you.
